PERSONAL DATA PROCESSING INFORMATION BY WROCŁAW MEDICAL UNIVERSITY

In accordance with Article 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data (General Data Protection Regulation, hereinafter referred to as GDPR), we inform you that:

PERSONAL DATA CONTROLLER

1. The administrator of personal data collected in connection with students’ use of forms available on the website https://ed.umw.edu.pl/ is Wrocław Medical University named after Piast Dynasty located at Pasteur’s Coast 1, 50-367 Wrocław, represented by the Rector.

2. In situations where personal data provided via forms are intended for the International Student Council in matters of:
a) representing the general student body of the University,
b) defending students’ rights,
c) activity in the field of student affairs, including social, welfare, and cultural matters,
d) striving to create the best possible conditions for student education and functioning,
e) expressing the opinions of the student community on matters important to students,
f) conducting informational activities by ISC,
The data controller will be the Student Government of Wrocław Medical University, which processes data to fulfill its tasks arising from applicable laws and Student Government Regulations.

DATA PROTECTION OFFICER

1. Wrocław Medical University has appointed a Data Protection Officer whom you can contact regarding personal data processing at: iod@umw.edu.pl

2. In cases where the Student Government is the controller of personal data, all questions and requests regarding the processing of personal data should be directed to the International Student Council via email: hanna.popiela@student.umw.edu.pl

PURPOSE AND LEGAL BASIS OF PERSONAL DATA PROCESSING

Personal data are collected and processed solely based on appropriate legal grounds, and the scope depends on the selected contact form and program in which you want to participate.

The dedicated form allows you to send messages to the International Student Council. Personal data provided in the form are collected for the purpose of ISC contacting students for cooperation or joining ISC and providing any related information.

The legal basis for processing personal data by Wrocław Medical University is Article 6(1)(c) and (e) of the GDPR in connection with the Act of 20 July 2018 – Law on Higher Education and Science, i.e.,

the necessity of processing to perform a legal obligation incumbent on the University, in particular, the obligation to provide conditions necessary for the functioning of the Student Government, including infrastructure,
performing a task carried out in the public interest, which includes conducting scientific activities, shaping civic attitudes, participating in social development, and creating an innovation-based economy.

2. Student affairs

The dedicated form allows you to send questions, concerns, complaints, and study-related problems to ISC or University authorities (dean, vice-dean). Personal data provided in the form are collected for ISC and University authorities to contact students for answering questions and addressing and taking action on matters requiring attention.

The legal basis for processing personal data by Wrocław Medical University is Article 6(1)(c) and (e) of the GDPR in connection with the Act of 20 July 2018 – Law on Higher Education and Science, i.e.,

the necessity of processing to perform a legal obligation incumbent on the University, in particular, the obligation to provide conditions necessary for the functioning of the Student Government, including infrastructure,
the necessity of processing for the proper and timely performance of the University’s tasks by ensuring communication with students and conducting informational activities on matters addressed to us.

3. Mentoring

The “Get mentored” form is intended for students who apply for a mentor among senior students. Personal data provided in the form are collected for the purpose of assigning a mentor and providing the mentee’s contact details to the mentor.

The “Become a mentor” form is intended for senior students who want to mentor younger students. Personal data provided in the form are collected for the purpose of assigning a mentee and providing the mentor’s contact details to the mentee.

The legal basis for processing personal data by Wrocław Medical University students is Article 6(1)(a) of the GDPR, i.e., voluntary consent to the processing of data expressed by submitting the selected form. Consent can be withdrawn at any time by sending an email to: yewai.yan@student.umw.edu.pl. Upon withdrawal of consent, your personal data will be immediately deleted from the Mentoring program.

4. Erasmus Become a buddy

The “Become a buddy” form is intended for students who want to assist foreign students coming to study at Wrocław Medical University under the ERASMUS program. Personal data provided in the form are collected to enable the selection of an incoming student from the list provided to you and simultaneously provide them with your contact details.

The legal basis for processing personal data by Wrocław Medical University students is Article 6(1)(a) of the GDPR, i.e., voluntary consent to the processing of data expressed by submitting the selected form. Consent can be withdrawn at any time by sending an email to: wanda.ambrosius@student.umw.edu.pl. Upon withdrawal of consent, your personal data will be immediately deleted from the Become a buddy program.

5. Chat with WMU International Student Council

Personal data provided during the chat will be processed to provide all information on questions asked by you.

The International Student Council (Student Government of Wrocław Medical University), as the founder of the fan page/messenger on Facebook, is the Administrator and at the same time a joint controller of personal data together with Facebook Ireland Limited. Each of the mentioned Administrators decides on the purposes and means of processing data through the fan page/messenger, but to a different extent. The Student Government is solely responsible for the processing of personal data collected for communication via messenger.

The legal basis for processing personal data by ISC is Article 6(1)(c) and (e) of the GDPR in connection with the Act of 20 July 2018 – Law on Higher Education and Science, i.e., the necessity of processing to perform a legal obligation incumbent on the Controller and performing a task carried out in the public interest, which includes conducting informational activities.

RECIPIENTS OF PERSONAL DATA

1. Wrocław Medical University does not disclose any personal data recipients related to students’ use of website functionality, except in cases where such obligation arises from universally applicable law.

2. Wrocław Medical University may entrust another entity, by written agreement, with the processing of personal data on its behalf, in particular, IT service providers.

PERIOD OF PERSONAL DATA STORAGE

1. Wrocław Medical University will retain personal data for the period necessary to achieve the aforementioned purposes or until you withdraw your consent to the processing of personal data or object to the processing. In such a case, personal data will be promptly deleted.

Personal data that Wrocław Medical University is obliged to keep in accordance with the regulations on documentation archiving will be processed until the expiry of the period indicated in these regulations.

INFORMATION ABOUT RIGHTS

In cases, on the terms, and in the manner specified in applicable regulations, students have the right to request:

  • access to personal data and their rectification (Articles 15 and 16 of the GDPR),
  • erasure of data (Article 17 of the GDPR),
  • restriction of processing (Article 18 of the GDPR),
  • objection to processing (Article 21 of the GDPR),
  • data portability (Article 20 of the GDPR),
  • withdrawal of consent to the processing of personal data at any time. Withdrawal of consent does not affect the lawfulness of processing based on consent before its withdrawal.

Persons whose data are processed also have the right to lodge a complaint with the supervisory authority – the President of the Office for Personal Data Protection located at Stawki Street 2, 00-193 Warsaw – if they suspect that personal data are processed by Wrocław Medical University in violation of the law.

INFORMATION ABOUT THE OBLIGATION TO PROVIDE DATA AND POSSIBLE CONSEQUENCES OF REFUSAL TO PROVIDE DATA

Providing personal data is not mandatory but necessary to use all website functionalities, such as applying for student programs or receiving necessary information. Refusal to provide data will prevent us from sending relevant information or inviting you to cooperate.

INFORMATION ABOUT AUTOMATED DECISION-MAKING, INCLUDING PROFILING

Personal data are not subject to automated decision-making, including profiling, referred to in Article 4(4) of the GDPR, which means automated processing of personal data consisting of using personal data to assess certain personal aspects of a natural person.
